DUI Services at the Lighthouse

So, you've received a DUI and aren't sure what steps to take in 

order to fulfill your court orders and state requirements?  

We are here to help you through this process with dignity, 

respect, and understanding.


If you are a resident of the state of Kentucky and receive a DUI, 

you are required to have a DUI Assessment. 

Even if you receive the DUI in a different state, all Kentucky residents 

are required to have

an Assessment completed by a Certified DUI Assessor. 


If you received your DUI in the state ofKentucky, you will need to bring 2 pieces of documentation in order to schedule your appointment.


1.  Copy of the Uniform Citation (ticket)


2.  AOC-494 Form ~ "Notice to Attend Alcohol Driver 

Education Program"

(you receive this notice at your court date)


If you received your DUI in the state of Ohio, you will only need to bring 

the Uniform Citation.


If you're unsure if you have these documents in your possession, you may call the 

County Clerk's office in the county

in which you received the DUI, and they can fax these documents 

directly to us at 606-834-0049.


Kentucky and Ohio DUI Assessment Fee: $90

West Virginia DUI Assessment Fee: $110

*Payment is REQUIRED prior to scheduling the Assessment.*

If paying by card, there is an additional $3 Card Processing fee



DUI Assessments/Treatment/Education courses are 

available and scheduled as a

one on one session with your counselor.  M-F, 9-5



DUI Treatment courses are $45 per session 

(as of 1/1/2020)



DUI 1st Offense- Normally requires a 

minimum of 8-10 sessions.

(However, this is to be determined during 

your initial assessment) 

OR referral to an education program.


(DUI 1st - you have 90 days from date of assessment to complete education or treatment program)


DUI 2nd Offense requires a 

MINIMUM 6 month treatment program, up to a year ~


**State requires that you may only attend ONE session per week**


Failure to attend scheduled sessions with no contact after 30 days 

will be considered

NON-COMPLIANT according to Kentucky regulations. 

If this happens, you MUST complete another Assessment

We must work closely together to successfully complete your program!


*Some insurance companies will allow payment of Treatment Courses*


(Groups may become available at a later day, if enrollment allows)
